52 WEEKS TO FINANCIAL ORGANIZATION: #20 – WMFNTK: Legal
Last week we began creating a document called "What My Family Needs to Know" (WMFNTK). Why do this? So that your family member or personal representative will be able to easily retrieve your personal, legal, financial, and medical information when you're not able to tell them where to find it. Remember, there is no "if I die".
This week, your homework is to create Chapter 2 of your WMNTK document: My Legal Information.
Below is a list of possible personal information that you might have. Copy and paste that list into a Word document. Then delete any items on the list that do not apply to you.
My Legal Information: (Who I Am and What I Want)
Birth Certificate: DOB, city & state, certificate #
Adoption Certificate: date of adoption, city & state
Marriage Certificate: date of marriage, city & state
Annulment Certificate: date of annulment, city & state
Divorce Certificate: date of divorce, city & state
Naturalization Papers: date of citizenship, certificate #
Military Discharge Papers: date of discharge, certificate #
Passport: date of issue, passport #
Driver's License: date of issue, driver's license #
Death Certificates of Close Relatives: location of certificates
Financial Power of Attorney: date created, name & contact info for POA
Medical Power of Attorney: date created, name & contact info for POA
Advance Directives: names of people who have copies
Living Will: names of people who have copies
Organ Donor? where your desire is documented
Will: date created, any codicils, attorney's contact info, executor's contact info
Trust(s): date created, attorney's contact info, trustees' contact info
Letter(s) of Instructions: location, names of recipients
Funeral & Burial Instructions: cemetery name, address & lot numbers, deed to cemetery lot, preferred recipient(s) of memorial gifts, desires for funeral ceremony, location of prepaid funeral services document
Fill in all applicable information, including the locations of the original documents. If you are preparing this information for other persons, as well as for yourself, create a separate record for each person.
Save a copy of your WMFNTK document to your hard drive, a CD, or a flash drive. Include the date in the file name, so that you will know when you created it.
Print a copy of the Legal section and insert it into your binder under the second tab.
